Depression self test

___          Feeling sad, discouraged or hopeless

___          Lack of interest in doing activities you enjoy

___          Lack of desire to be with your friends

___         Excessive self blame

___         Persistent thoughts about dying
 
If more than one statement is true of you,
there might be something to talk about.
 

Anxiety self test
How many statements apply to you right now?
 
___        Worrying a lot about more than one thing, such as
              making a mistake, being abandoned, getting sick       
 
___        Being on edge a lot, waiting for something bad to happen
 
___        Difficulty concentrating
 
___        Problems with sleep
 
___        Being easily fatigued

___        Physical symptoms associated with worry
 
___        Irritability, or being easily annoyed by small things
 
___        Having difficulty comforting yourself when you get upset
